Traditional Independent Medical Exam
This is a thorough case review and report. Prior to the interview the employee is required to complete an intake form which includes a pain diagram and information related to the mechanism of the injury/illness. The physician will review all available medical records and radiographs and take a complete history from the employee. An examination will be completed of the affected body part(s) and related areas.
The report includes:
- History of the Present Illness
- Past History (Surgical, Medical & Orthopaedic)
- Family History
- Social History (including Work History)
- Review of Symptoms (when appropriate)
- Physical Examination
- Impression
- Recommendations

Chart Reviews
This is a record review and report. The physician will review all available medical records and radiographs.
This type of service may be used to screen a case for appropriateness of care or to determine if a Traditional IME is necessary.
The report includes:

Impairment Exams
This is a brief case review and report. Prior to the interview the employee is required to complete an intake form which includes a pain diagram and information related to the mechanism of the injury/illness. The physician will review all available medical records and radiographs and take a brief history from the employee. An examination will be completed of the affected body part(s) and related areas.
This type of service may be completed after the employee has reached Maximal Medical Improvement (MMI). The employees functional limitations will be rated using the American Medical Association's Guides to the Evaluation of Permanent Impairment, 5th Edition.
The report includes:
- Brief History of the Present Illness
- Physical Examination
- Impression
- Impairment Rating

Causal Opinion
Usually done in combination with a Traditional IME.
This type of service may be completed when there is a question as to the work-relatedness of a medical condition. The physician will focus specifically on the mechanism of injury and determine if causal factors are present in the work environment to result in the medical condition.
The report will state whether causal factors exist to support/not support the medical condition. Justification for the decision is provided. At times medical references are quoted.
Job Site Visits may be required to better understand the environment of the injury.

Fitness For Duty
This type of service may be completed when the employer is concerned about the ability of an employee to complete job task given a recent history of a medical condition. The physician will focus specifically on the recent medical condition and determine if the employee has reached the point of Maximal Medical Improvement (MMI). The physician may be required to review medical records and radiographs and confer with the treating physician. An examination will be completed of the affected body part(s) and related areas.
The report includes:
- History of the Present Illness
- Past History (Surgical, Medical & Orthopaedic)
- Family History
- Social History (including Work History)
- Review of Symptoms (when appropriate)
- Physical Examination
- Impression
- Recommendations
The report will state whether return to work is recommended given the medical condition. Justification for the decision is provided.
Job Site Visits may be required to better understand the environment to which the employee is returning.

Permanent Restriction Opinion
This is a brief case review and report. Prior to the interview the employee is required to complete an intake form which includes a pain diagram and information related to the mechanism of the injury/illness. The physician will review all available medical records and radiographs and take a brief history from the employee. An examination will be completed of the affected body part(s) and related areas.
This type of service may be completed after the employee has reached Maximal Medical Improvement (MMI). The physician will provide a written opinion on the type of restrictions, if necessary, to appropriately place an employee at the workplace.
The report includes:
- Brief History of the Present Illness
- Physical Examination
- Impression
- Permanent Restrictions
The availability of a Job Description is important in determining the functional demands required for a job. At times a Job Site Visit may be necessary to better understand the work environment.
